➡️We got a call from the answering service tha ➡️We got a call from the answering service that a pregnant patient was coming in and the comment was “she says her uterus is falling out.”
Us - 🤔🤔🤔
She comes in and the midwife sees her. I get a call shortly thereafter and she says…..
“the patient’s cervix is hanging out” 😲
➡️I go see the patient and indeed her cervix is hanging slightly out of her vagina!
She has something called pelvic organ prolapse in pregnancy, a condition that affects 1 in 10,000 to 1 in 15,0000 pregnancies.
➡️I can recall seeing it one other time in over 15 years. This was her 3rd pregnancy and both of the other 2 were cesarean births. Contrary to popular belief, cesarean does not prevent prolapse.
➡️For the immediate treatment I gently pushed her cervix and uterus back into her vagina.
The longer term treatment is something called a pessary, a rigid device inserted into the vagina that supports the cervix and uterus.
➡️Those take time to get and require fitting by someone who is trained in fitting those (usually a urogynecologist).
If the prolapse happens again before she gets the pessary, then she can wash her hands and gently push her cervix and uterus back in herself.
I tell you, there is NEVER a dull day in my line of work 🙃.
